NEW Patients: 

 When calling to schedule an appointment please set aside approximately 15 minutes for the intake process when calling.

  1. We will need all demographics to include full name, date of birth, address, cell phone number, and an email address is required. 
  2. We will need a list of all current medication to include dosage and instructions for each medication.
  3. We will need to know what pharmacy you want to have prescriptions sent to and the address of the location of your choice (phone number if available)
  4. Once scheduled you will complete all consents and intake forms through our patient portal that you will receive information on
Please have all of the above information available when calling to schedule your appointment. It is patient responsibility to update all contact and demographic information upon any changes by calling our office and either speaking with someone or leaving a detailed voicemail with the information that needs to be updated.
